Regulation .02 Modified Diet.

If an operator agrees to accept a child who requires a modified diet for:
A.
Medical reasons, the operator shall obtain from the child's parent a written prescription for the diet signed and dated by the child's licensed health practitioner within the previous 6 months; or
B.
Cultural or religious reasons, the operator shall obtain written, dated instructions for the diet signed by the child's parent.
