Regulation .14 Annual Reporting.

Each licensee shall submit an annual report to the Department on a form designated by the Department.

This report shall be submitted by March 1 of each calendar year and information provided will summarize fertilizer applications in the previous calendar year.

Failure to submit this report by March 1 may result in the suspension of a business license or professional fertilizer applicator certificate.
