Whenever a party is required by this chapter to serve a copy of a pleading or memorandum upon another party, a certificate shall be appended to the pleading or memorandum stating: To whom the copy was sent; The address to which the copy was sent; The name of the person who sent the copy; The date the copy was sent; and Manner of service.
