Application Completion Criteria. Applications for a scrap tire hauler license are considered complete only if they are: Submitted on application forms provided by the Department; Accompanied by all required exhibits and supporting documents, as applicable; and Completed in full and signed by the applicant. Upon receipt of a completed application, the Department may deny the license if the: Application contains a material misrepresentation or false information, or fails to fully disclose relevant facts; and Applicant has not complied with these regulations, or other applicable statutes and regulations of the Department or other governmental agencies.
