A.
Each commercial ambulance service proposal or contract for services shall contain the:
(1)
Designation "Md.Comm.Amb.Lic.#" followed by the ambulance service license number issued by MIEMSS; and
(2)
Following notice:

Each commercial ambulance service invoice for services shall contain the designation "Md.Comm.Amb.Lic.#" followed by the ambulance service license number issued by SOCALR.
