A commercial ambulance service shall maintain complete and accurate records regarding the care and transportation of each patient that the commercial ambulance service:
Responds to transport;
Agrees to transport; or
Transports to, from, or within the State.
A record for each patient referred to in §A of this regulation shall contain the following information:
Patient's name;
Patient's residence address;
Date and time of scheduled pickup;
Location of pick up;
Date of service;
Time dispatched;
Commercial ambulance number;
Time en route to pick up location;
Time arrived at pick up location;
Time en route to destination;
Patient's destination;
Time arrived at destination;
Referring physician;
Receiving physician, if available; and
Pertinent remarks.
A commercial ambulance service shall use the standard Maryland Ambulance Information System runsheet and additional narrative supplied at cost by SOCALR.
