A provider with a certificate, including a certificate issued before the adoption of these regulations, may apply to have the certificate renewed. An updated application for renewal shall be submitted on forms approved by the Department. An application for renewal shall be filed with the Department by a date specified annually by the Department. The Department shall renew a certificate if: The provider is in compliance with this chapter; and The Department has verified that the requirements of Regulation .03 of this chapter have been met.
