An individual required to provide information under Regulation .02 of this chapter shall also submit a completed background form supplied by the Commission which includes a statement disclosing whether the individual has ever been: Arrested; Convicted of, pled nolo contendere to, or received probation before judgment for, a felony or misdemeanor, other than a misdemeanor traffic offense; Sanctioned by a government agency related to gaming; Found liable in connection with a civil action related to gaming; A debtor in a bankruptcy proceeding; or Denied a bond.
